Please do not submit protected health information (PHI) through this website. Our website forms are for business contact and scheduling only. Do not enter patient information, passwords, login credentials, or confidential system details. If you need to share sensitive information, we will arrange a secure method during your engagement.

We collect the information you choose to provide when you complete a form or contact us. This typically includes your name, business email address, phone number, job title, organization name, and general location (city and state), along with any message you send us.

We do not intentionally collect protected health information through this website, and you should not submit it here.

Service Providers

We use a small number of trusted providers to operate this site and deliver our services. They process information only as needed to provide their service to us:

  • Netlify hosts this website and processes website form submissions.
  • Jotform powers our onboarding, intake, and consultation-request forms.
  • Stripe processes payments. Payment card details are handled directly by Stripe; we do not receive or store full card numbers.
  • Calendly and Microsoft 365 support scheduling and email communication.

Because these forms are intended for business contact and demographic information only, they are not designed to receive protected health information.

Cookies and Tracking

We use only the essential cookies needed for the site to function, plus, with your consent, Google Ads cookies that let us measure the performance of our advertising. When you first visit, our cookie banner lets you accept or reject these non-essential cookies, and we honor your choice, including recognized browser opt-out signals.
